Referrals

A detailed referral from your GP, physiotherapist or other medical practitioner is required.

​Every referral is individually assessed and prioritised according to urgency. Some referrals provide enough detail to allow further investigations to be arranged prior to your appointment.

​If your condition is deemed urgent, it may be re-directed as appropriate so that you receive the care you need in an appropriate timeframe.

For ACC cases, we require ACC claim details, including claim number and date of injury.

​ Self-referrals are unable to be accepted.
